无托槽隐形矫治器与固定矫治器对口腔正畸患者牙周状况的影响观察

目的分析口腔正畸患者应用固定矫治器与无托槽隐形矫治器矫治后的牙周状况。方法94例口腔正畸患者,按随机数字表法分为A组与B组,各47例。A组给予传统固定矫治器治疗,B组使用无托槽隐形矫治器治疗。比较两组相关临床指标、牙周健康指数、并发症发生情况。结果B组倾斜牙矫正、转位牙扭正及完成治疗时间分别为(3.60±0.77)、(4.06±0.49)、(9.77±1.35)个月,均短于A组的(6.16±0.31)、(7.14±0.62)、(14.34±0.19)个月,差异有统计学意义(P<0.05)。治疗前,两组牙龈指数(GI)、龈沟探诊深度(SBI)、龈沟出血指数(SPD)、菌斑指数(PLI)比较,差异无统计学意义(P>0.05)。治疗后,B组PLI低于A组,差异有统计学意义(P<0.05);两组GI、SBI、SPD比较差异无统计学意义(P>0.05)。B组并发症发生率4.26%低于A组的19.15%,差异有统计学意义(P<0.05)。结论无托槽隐形矫正器与传统固定矫正器相比,其正畸效率更高,对患者牙周健康的影响小,临床上值得推广使用。

Observation on the influence of bracketless invisible appliance and fixed appliance on periodontal condition of orthodontic patients

Objective To analyze the periodontal condition of orthodontic patients after applying of bracketless invisible appliance and fixed appliance.Methods A total of 94 cases of orthodontic patients were divided into group A and group B according to random numerical table,with 47 cases in each group.Group A was treated with traditional fixed appliance,and group B was treated with bracketless invisible appliance.The related clinical indicators,periodontal health index and occurrence of complications were compared between the two groups.Results The time for correction of tilted teeth,twisting of transposed teeth and completion of treatment in group B were(3.60±0.77),(4.06±0.49),(9.77±1.35)months,respectively,which were shorter than(6.16±0.31)and(7.14±0.62),(14.34±0.19)months of group A,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in gingival index(GI),sulcus bleeding index,sulcus probing depth(SPD)and plaque index(PLI)between the two groups(P>0.05).After treatment,PLI of group B was lower than that of group A,and the difference was statistically significant(P<0.05).There was no statistically significant difference in GI,SBI and SPD between the two groups(P>0.05).The incidence of complications 4.26%of group B was lower than 19.15%of group A,and the difference was statistically significant(P<0.05).Conclusion Compared with the traditional fixed appliance,bracketless invisible appliance has higher orthodontic efficiency and less influence on periodontal health of patients,which is worthy of clinical promotion and application.
