高血压病患者肾血流的变化及海捷亚对其的影响

摘    要:目的在抗高血压药物海捷亚的干预下,用彩色多普勒血流测定的方法评价高血压病患者肾血流的变化.方法Ⅱ期高血压病30例,口服海捷亚1片/日,共4周.服药前及服药后第二、第四周末用彩色多普勒方法测定肾血流.结果用药四周后血压均显著下降,肾内血流由低速高阻型变为高速低阻型.段动脉和叶间动脉的收缩期最大血流速度(Vs)、舒张期末最低血流速度(Vd)、血流峰值加速时间(AT)、阻力指数(RI)和脉冲指数(PI)均较用药前明显改善(P<0.01,P<0.05).血压降低与肾血流参数之间呈明显直线相关.结论Ⅱ期高血压患者即有肾血流的降低,海捷亚在降压的同时可改善肾血流.彩色多普勒肾血流测定可作为早期评价高血压患者肾脏受累和抗高血压药肾脏保护作用的有效方法.

Abstract:Objectives To observe the alteration of renal blood flow in hypertensive patients and evaluate the influence of Hyzaar(Losartan 50mg+Hydrochlothizide 12.5mg)on the renal blood flow of the patients with the Color Doppler Flow Imaging. Methods All the 30 cases of essential hypertension were given Hyzaar 1 tablet per day for four weeks. At the end of second and fourth week,renal blood flow was measured with Color Doppler. Results All the patients' blood pressure lowered apparently after four weeks' treatment with Hyzaar. The Vs,Vd,PI,RI and AT improved apparently at the end of the fourth week.Linear correlation was found between the blood pressure lowered and the improvement of the parameters of renal blood flow. Conclusions The renal blood flow reduced in essential hypertensive patients even with a normal renal function. Hyzaar not only lowered blood pressure,but improve the renal blood flow also. Color Doppler Flow Imaging can be used to evaluate the early alteration of the renal blood flow in hypertensive patients.
