心脏瓣膜置换术后应用抗凝药物的护理干预和观察

目的:探讨心脏瓣膜置换术后应用抗凝药物的护理干预措施和效果。方法:对60例机械瓣膜置换术患者应用抗凝药物,进行护理干预和健康教育,定期复查PTT、PTA,严密观察,以防用药不当出现血栓或出血等并发症。结果:60例患者未发生栓塞,2例出现鼻衄,1例出现便血,通过调整药量症状均消失。定期复查凝血四项数据波动小,相关并发症出现晚,患者再次入院时间明显推迟。结论:采用针对机械瓣膜置换术后系统科学的抗凝护理干预措施,实现患者及其家属的主动配合并学会参与共同完成抗凝监测。

NURSING INTERVENTION AND OBSERVATION FOR PATIENTS APPLIED ANTICOAGULANT AFTER CARDIAC VALVE REPLACEMENT

Objective:To discuss the measure and effect of nursing intervention and observation for patients applied anticoagulant after cardiac valve replacement.Methods:Nursing interven-tion and health education for 60 patients applied anticoagulant after cardiac valve replacement,regular review PTT、PTA to avoid syndrome of valve embolism and bleeding.Results:60 patients had not embolism,symptoms disappeared through adjusting medicine dose on epistaxis of 2 patients and hemafecia of 1 patient. Time of blood coagulation fluctuation within a narrow range,complications developed late,second time of patients hospitalization put off.Conclusion:It can realize patient and family initiative cooperation and participation to monitor anticoagulation through adopting nursing intervention of system science for patients applied antico-agulant after cardiac valve replacement.
