血清孕酮诊断早期异位妊娠的意义

目的:探讨血清孕酮(P)对异位妊娠(EP)早期诊断的临床意义。方法:回顾性分析2010年7月—2011年4月本院EP患者100例中,采用化学发光分析法测定其血清孕酮值,与同期100例因先兆流产就诊而被证实为宫内早孕者血清孕酮值进行比较分析。结果:异位妊娠患者血清孕酮值为(6.12±5.23)nmol/L,因先兆流产就诊而被证实为宫内早孕者为(15.31±4.37)nmol/L,两者比较差异有统计学意义(P<0.01)。选择15nmol/L为血清孕酮临界值时,异位妊娠与正常宫内妊娠的鉴别诊断中,阳性预测值为88.4%,阴性预测值为91.7%,敏感性为92%,特异性为88%。结论:血清孕酮检测简便、快捷,诊断率较高,可作为临床上早期诊断异位妊娠的重要辅助手段,从而有助于异位妊娠及时诊断与治疗,降低风险。

The Value of Serum Pregestevore in the Early Diagnosis of Ectopic Pregnancy

Objective:To explore the value of serum progesterone in the early diagnosis of ectopic pregnancy.Methods:Retrospective analyse clinicaldata of 100 patients with suspected EP of ourhospital from July 2010 to April 2011,The determination of chemiluminescence analysis the serum progesterone value,Compared with the 100 cases of threatening miscarriage clinic and proves to be a palace the results serum progesterone value is used in the comparison analysis.Results:Progesterone in the ectopic pregnancy group was significantly lower than that in the control group(6.12±5.23)nmol/L vs(15.31±4.37)nmol/L,P<0.01,when you choose the serum progesterone critical value is 15nmol/L,Ectopic pregnancy and normal intrauterine pregnancy in the differential diagnosis,positive predictive value is 88.4%,Negative predictive value is 91.7%,sensitivity is 92%,specificity is 88%.Conclusion:Serum progesterone detection is simple and fast,diagnosis rate is higher,and it Can be used as a clinical diagnosis of early ectopic pregnancy important auxiliary measures.Thus it can help to diagnosis and treatment the ectopic pregnancy timely.
