达克普隆治疗46例胃巨型溃疡临床分析

目的　了解胃巨型溃疡发病情况、发生部位及质子泵抑制剂治疗效果。方法　 2 0 0 1年 5月～ 2 0 0 3年 5月期间我院胃镜室在门诊及住院病人中经内镜诊断的 46例胃巨型溃疡给予达克普隆胶囊及胃黏膜保护剂治疗 ,根据临床表现、溃疡大小、形态、发生部位以及质子泵抑制剂达克普隆胶囊的疗效进行分析。结果　胃巨型溃疡较平常胃溃疡位置高 ,多在胃体部 ;胃巨型溃疡形态以圆形、椭圆形为主 ,溃疡底大多深凹 ;大多数伴有中至重度浅表性胃炎或萎缩性胃炎 ;胃巨型溃疡发病年龄以 40～ 5 0岁居多。结论　胃巨型溃疡不一定都是恶性 ;当镜下诊断胃巨型溃疡与常规病理诊断不相符合时 ,应该短期内行进行内镜复查 ,同时可佐以X线检查以排除恶性溃疡 ;质子泵抑制剂治疗胃巨型溃疡疗效确切。

Clinical Analysis of 46 Patients with Giant Gastric Ulcer Treated with Lansoprazole

Objectives To investigate the morbidity and location of giant gastric ulcer as well as inhibitory effect on the disease with proton pump inhibitor(PPI). Methods We chose 48 patients with giant gastric ulcer confirmed by endoscopy in our hospital from May 2001 to May 2003, and the patients were treated with landsoprazole and protective drugs of gastric mucosa respectively. Then we analyzed the clinical manifestations of each patient and the size, shape as well as location of each ulcer, in addition, the effect of treatment with landsoprazole was also estimated. Results Most giant gastric ulcer was usually located on gastric corpus, with round or ellipse shape, deep and giant base, and accompanied with moderate or heavy superficial gastritis or atrophy gastritis. Most patients with giant gastric ulcer were forties or fifties. Conclusions Our data revealed that giant gastric ulcer was not always malignant. When the nature of the ulcer was suspectable, it should be examined by endoscopy as soon as possible, additionally, upper gastrointestinal radiography could be also helpful. PPl was effective in the treatment of giant gastric ulcer.
