经脐单孔腹腔镜胆囊切除术的临床体会

目的:探讨经脐单孔腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的可行性及操作要点。方法:回顾分析为18例患者行经脐单孔LC的临床资料,术中使用三通道穿刺器及可弯曲手术器械。结果:15例成功施行经脐单孔LC,3例因胆囊三角区解剖不清增加一个切口。术后1例发生脐部切口疝,余者均无胆管损伤、术后出血等手术相关并发症发生。结论:经脐单孔LC安全可行,并具有美容及更加微创的优点,但技术要求较高,术者在熟练掌握腹腔镜手术技巧的基础上可开展此术式。

The clinical experience of transumbilical single-port laparoscopic cholecystectomy

Objective:To explore the feasibility and operative skills of transumbilical single-port laparoscopic cholecystectomy(LC).Methods:The clinical data of 18 patients who were treated with transumbilical single-port LC were retrospectively analyzed.Three-channel Trocar and flexible apparatus were used during the operations.Results:Three cases were converted to two-port LC because of adhesion in Calot's triangle.Other fifteen cases were performed transumbilical single-port LC successfully.No intraoperative complications such as bile duct injury or postoperative bleeding occurred,except transumbilical incisional hernia happened in one patient.Conclusions:Transumbilical single-port LC is a safe,feasible and more minimally invasive technique with better cosmetic results.This operation can be widely applied in clinic when the surgeons proficiently master the correct operative skills.
