阳虚水泛型肺胀患者血液动力学和右心时相的研究

摘    要:
本文运用肺阻抗血液图(IPR)、三尖瓣超声心动图右心时相(TV—e-cho RVTI)和右心导管术对22例阳虚水泛型肺胀患者、21例肺肾气虚型肺胀患者肺循环血液动力学和右心时相进行了对比研究,结果肺动脉平均压阳虚水泛型明显高于肺肾气虚型,肺动脉顺应性明显下降,REML、RIRT、C—D段、RPEP、RICT延长、RDTI缩短,RSTI/RDTI、RIRT/RDTI-RIRT、RPEP/RVET及RVET/RICT比值增大,其组问差异均具有统计学意义.表明阳虚水泛型肺胀患者肺动脉顺应性下降,肺动脉压增高、右心功能受损.

Abstract:
The authors used IPR, TV-echo RVTI and right cardiac catheterization to measure the hemodynamic indexes and right ventricular time intervals in 22 patients with lung distension caused by edema of Yang deficiency and 21 cases caused by deficiency of kidney Qi and made the comparative study between two groups. Results showed the average pressure of pulmonary artery in edema of Yang deficiency group was much higher than that in deficiency of kidney Qi group,and also in patients caused by edema of Yang deficiency, the compliance of pulmonary artery was remarkably reduced, REML, RIRT, C-D segment, PREP and RICT were prolonged, RDTI was decreased and RSTI/RDTI, RIRT/RDTI-R1RT, RPEP/RVET and RVET/RICT were increased. There is statistical difference between two groups. All above results indicate that the patients of lung distension caused by edema of Yang deficiency have a decrease in compliance of pulmonary artery,an increase of the pressure of pulmonary artery and the damage of the function of the right heart.
