脊髓分级压迫损伤病理组织学改变与磁刺激运动诱发电位相关性研究

摘    要:目的:探讨脊髓分级压迫损伤的病理改变与磁刺激诱发电位之间的相关性.方法:选用41只成年家猫,随机分为对照组,轻、中、重度损伤4组.应用磁刺激运动诱发电位(MEPs)对脊髓压迫前、压迫后30min,6h,1wk,2wk及4wk做动态观察,观察血管、神经细胞、神经纤维及中央管的病理改变.结果:轻和中度损伤的病变是可逆的,而重度损伤导致神经细胞的不可逆改变和运动功能丧失.轻度损伤MEPs潜伏期在损伤后30min及6h比术前分别延长0.37倍和0.38倍,至4wk复原;中度损伤MEPs潜伏期延长0.77倍和0.81倍,重度组延长1.322倍和1.361倍.中度损伤组有部分恢复而重度组无明显变化.结论:脊髓分级压迫损伤病理组织学变化与运动功能和MEPs之间在统计学上具有明显相关性.

Abstract:AIM: To determine the pathological changes (PC) of the spinal cord caused by injury and their correlation with values obtained by the Magnetic Motor Evoked Potentials (MEPs) technique. METHODS: We studied spinal cords from 41 adult cats that were divided into 4 groups. The groups ranged from normal cats whose spinal cords were not compressed, to slightly, moderately and severely injured. MEPs were recorded before compression, and at 30 min, 6 h, 1 wk, 2 wk and 4 wk after the compression unit was installed. Pathological changes with increased pressure were observed in blood vessels, neural cells and fibers, and the central canal. RESULTS: A reversal of pathological changes was observed in slight or moderate injury during the 4 wk of the experiment. Extensive injury, however, caused irreversible changes in the nerve cells with loss of motor function. The latency of MEPs at 30 min and 6 h in the slightly injured group was 0.37 and 0.38 times greater than the baseline and returned to normal levels at 4 wk. In the moderately injured group, the latency increased 0.77 and 0.81 times and in the severely injured 1.32 and 1.361 times over the baseline. Partial recovery was found in the second group and no recovery in the severely injured. CONCLUSION: There appears good correlation between the observed pathological changes, motor functions and MEPs.
