复合麻醉无痛分娩应用于潜伏期的临床观察

目的：评价复合麻醉无痛分娩应用在第1产程潜伏期的镇痛效果，以及对母婴的影响。方法：选取本院2011年1月-2013年1月420例确诊无内科疾病、足月单胎初产妇纳入研究，随机将产妇分为A、B两组，每组各210例。A组在潜伏期行分娩镇痛，B组在活跃期时行分娩镇痛。观察比较两组的剖宫产率、新生儿Apgar评分、第1产程时间、潜伏期和活跃期镇痛效果、产妇对全产程的满意率及产后抑郁发生率。结果：（1）两组的剖宫产率、新生儿Apgar评分比较差异无统计学意义（P〉0.05）；（2）两组的第1产程时间、潜伏期和活跃期镇痛效果VAS评分、全产程满意率比较差异均有统计学意义（P〈0.05）；（3）两组产后抑郁发生率均较低。结论：伊捷卡硬膜外麻醉复合布托啡诺静脉注射分娩镇痛应用于产妇产程潜伏期时，分娩镇痛作用明显，产妇对全产程满意率高，对新生儿Apgar评分无影响，而且两组产后抑郁发生率均较低。

Application of Compound Anesthesia in the First Stage Latency

Objective：To evaluate the effectiveness and effect on maternal and infant of compound anesthesia in the first stage latency. Method：Four hundred and twenty healthy singleton term primiparas from January 2011 to January 2013 in the hospital were randomly divided into group A and group B,each group of 210 cases. Those in group A were treated by labor analgesia in the first stage latency,and those in group B were treated by labor anesthesia the active stage of labor. The indicators between the two groups were compared after treatment,such as the cesarean delivery rates,apgar scores of infants,the first stage times,analgesic effects,maternal comfort during total labor course and incidence of postpartum depression. Result：（1）There were no significantly different in cesarean delivery rates and Apgar scores of infants between two groups（P〉0.05）.（2）There were significantly different in analgesic effect VAS scores and maternal comfort during total labor course（P〈0.05）.（3）The incidences of postpartum depression in two groups were low. Conclusion：Levobupivacaine Hydrochloride epidural anesthesia combined with Butorphanol intravenous injection are effective in the first stage latency,have more maternal comfort during total labor course,no impact on Apgar scores of infants and a low risk of postpartum depression.
