摘    要:
应用腹腔镜检抽吸40例不孕症妇女腹腔液,测定其17β-雌二醇(E_2)孕酮(P)并与血液中E_2、P含量进行比较分析。结果显示在黄体早期,腹腔液中E_2、P浓度较血液中高5~10倍并持续6天。镜下见及末见排卵斑周期腹腔液中E_2、P浓度差异有极显著意义。以腹腔液E_2≥68.12pmol/L和P≥11.08nmol/L为排卵标准,诊断未破裂卵泡黄素化综合征(LUFS)15例(占38%)。以腹腔液E_2和P与同期血液E_2、P浓度比值≥5,诊断LUFS12例,(占30%),两种方法诊断符合率为80%。认为腹腔镜检同时测定腹腔液与血液中E_2、P浓度、结合子宫内膜病检是目前诊断LUFS较实用和可靠的方法。

Abstract:
In 40 women with infertility peritoneal fluid (PF)was aspirated under laparoscopy and levels of steroid hormones in PF and in plasma were measured. During early luteal phase,the PF 17beta-oestradiol(E_2) and progesterone (P)concertrations were approximately 5~10 times higher than those in plasma,respectively, which lasted for 6 days. The steroid hormone concertrations in PF were significantly higher in women with a detected stigma under laparoscopy than without stigma. The concertrations of E_2≥68.12pmol/L and P≥ll.18nmol/L in PF, were seen in 88% of women with stigma, whereas in those without stigma only 9% and 26% showed such concertrations. According to these criteria in the accuracies diagnosis of ovulation or luteinized unruptured follicle syndrome (LUFS) were 62% and 38% respectively. The concertrations of E_2 and P in PF were five times higher than serum concertrations in 70% normal women,30% LUFS women. The accuracy in diagnosis of LUFS by these two methods was 80%. In conclution, the measurement of E_2 and P in PF and in plasma is an effective method in LUFS diagnosis.
