全髋关节置换术后髋关节脱位(附6例分析)

目的：探讨人工全髋关节置换术后脱位的原因及最佳对策。方法：对我科在1995年1月至2001年12月治疗的全髋关节置换术后脱位6例进行回顾性分析。结果：6例中，脱位距手术时间最短1天。最长20天，平均10天，脱位假体类型。非骨水泥型假体4例，骨水泥型假体2例。脱位原因，髋臼假体角度不良4例，骨水泥技术不良1例。运送不当1例，脱位后治疗方法，徒手复位后髋“人”字石膏固定1例，下肢皮牵引1例，切开髋臼翻修4例，随访6个月-7年，平均3.5年。根据疼痛，生活能力，关节活动度，行走距离及X线片特征，进行功能评价，优良率83.33%。结论：最佳对策是正确安放髋臼假体的位置和角度，采用现代骨水泥技术和运送病人的安全性。

Hip joint dislocation after total hip replacement:Analysis of 6 cases

Objective:To investigate the reason for and treating method of hip joint dislocation(HJD) after total hip replacement(THR).Methods:Six cases of HJD after THR in my department from Jan.1995 to Dec.2001 were revewed.Results:There were six cases occurring HJD.The shortest time to operation was one day;the longest time was 20 days,averaging 10 days.The reasons of HJD included the bad angle of acetabular cups,spoiled quality and unsuitable transportation.Treatments included one case corrected dislocation of back hipbone by hands,and fixed with gypsum,one case of skin traction of crura,and four cases cut opened and rebuilt.Conclusions:The best method is placing the acetabular cups properly,adopting modern bone cement technology,and transportation patient safety.
