多囊卵巢综合征患者妊娠早期凝血和纤溶系统变化

目的探讨多囊卵巢综合征（polycystic ovary syndrome,PCOS）妇女与正常健康妇女妊娠早期凝血和纤溶系统指标的变化。方法收集2011年4月-2012年5月海军总医院产科门诊常规产检的孕妇,PCOS妊娠组50例,健康妊娠组50例。检测两组间凝血与纤溶系统指标的变化。结果与正常健康妇女妊娠早期相比,凝血因子Ⅷ（FⅧ）、凝血因子X（F X）的活性和血友病因子（vWF）、纤溶酶原活化剂抑制物（PAI-1）、组织型纤溶酶原活化剂（t-PA）的水平在PCOS妊娠早期组明显升高（P〈0.05）,但是D-D二聚体没有明显变化（P〉0.05）。结论 PCOS妊娠早期,FⅧ、vWF、F X可以较准确的预测高凝血状态。

Coagulation and fibrinolytic markers in first trimester of pregnant women with polycystic ovary syndrome

Objective To study the coagulation and fibrinolytic markers in the first trimester of pregnant women with polycystic ovary syndrome（PCOS） and normal pregnant women.Methods One hundred pregnant women who visited the Outpatient Department of Obstetrics and Gynecology,Navy General Hospital of Chinese PLA,from April 2011 to May 2012,were divided into PCOS pregnant group（n=50） and normal pregnant group（n=50）.Their coagulation and fibrinolytic markers were detected.Results The activity of coagulation factor Ⅷ（FⅧ） and coagulation factor X（F X） and the levels of von Willebrand factor（vWF）,plasminogen activator inhibitor 1（PAI-1）and tissue plasminogen activator t-PA were significantly higher in PCOS pregnant group than in normal pregnant group（P0.05）.However,no significant difference was observed in D-dimer level between the two groups.Conclusion The levels of FⅧ,vWF and F X can accurately predict the hypercoagulable state in the first trimester of pregnant women with PCOS.
