钬激光前列腺切除与经尿道前列腺汽化电切对BPH的疗效比较

目的 :比较钬激光前列腺切除术 (HoLRP)和经尿道前列腺汽化电切术 (TUEVP)对有症状前列腺增生症(BPH)的治疗效果。方法 :分别采用HoLRP和TUEVP对 2 4 8例BPH进行治疗。结果 :两组手术前后国际前列腺症状评分 (IPSS)、生活质量评分 (QOL)、最大尿流率 (Qmax)、剩余尿 (RUV)比较均得到显著改善 (P <0 .0 0 1 ) ;两组比较无显著性差异 (P >0 .0 5)。HoLRP组手术时间较TUEVP组长 (P <0 .0 0 1 ) ,术中出血HoL RP组较TUEVP组少 (P <0 .0 0 1 ) ,术后留置尿管时间、术后住院时间及术后并发症HoLRP组较TUEVP组明显减少 (P <0 .0 0 1 )。术后阳痿发生率 :HoLRP组为 1 .2 % ,TUEVP组为 7.7% (P <0 .0 5)。结论 :HoLRP能达到与TUEVP完全相同的治疗效果 ,且并发症少 ,近期疗效满意。是目前BPH激光治疗中较有发展前途的一种新技术

A COMPARATIVE STUDY ON THE EARLY RESULTS OF HOLMIUM LASER AND TRANSURETHRAL ELECTROVAPORIZATION RESECTION OF PROSTATE FOR BPH

Objective:To compare the therapeutic effects of the holmium laser and transurethral electrovaporization resection of prostate for the treatments of symtomatic benign prostatic hyperplasia(BPH). Method:Two hundred and forty-eight cases of BPH were treated with holmium laser and transurethral electrovaporization resection of prostate.Results:IPSS score, QOL, Qmax and RUV were significantly improved after either of the procedures and therapeutic results ( P <0.001);No significant difference in the improvement of subjective symptoms and objective signs has been noted with the different procedured ( P >0.05) .The resection time was longer for HoLRP than TUEVP ( P <0.001), but the interoperative bleeding was less for HoLRP than TUEVP ( P <0.001) .With respect to catheterization time ,hospital stay time and postoperative complications were less for HoLRP than TUEVP ( P <0.001) .The incideased of impotence in HoLRP group has been 1.2% and in TUEVP group 7.7% ( P <0.05).Conclusion: HoLRP has the same therapeutic effect as TUEVP and HoLRP less adverse side effects, it was a newly technology for the treatment symptomatic BPH.
