泻白清肝饮治疗热哮证临床疗效观察

目的观察泻白清肝饮治疗支气管哮喘热哮证的临床疗效。方法将2010年7月~2013年12月期间,支气管哮喘发作期热哮证患者共72例,随机分成两组,每组36人。两组在实施基线治疗基础上,治疗组口服泻白清肝饮,对照组口服氨茶碱片。统计两组临床疗效,比照治疗前后主要症状体征改善情况,嗜酸性粒细胞及白细胞水平变化情况。结果两组总有效率差异无统计学意义(P0.05)。治疗组在咳嗽及咯痰改善方面疗效显著好于对照组(P0.05)。与治疗前比较,两组治疗后嗜酸性粒细胞计数均显著改善,但组间比较差异无统计学意义(P0.05);治疗组白细胞计数与治疗前比较显著下降(P0.05),对照组白细胞计数与治疗前比较,无显著改善(P0.05)。两组无不良反应发生。结论泻白清肝饮可显著改善咳嗽、咯痰症状,通过降低哮喘患者急性发作期外周血中异常升高的嗜酸性粒细胞计数、白细胞数,减轻气道炎症及气道高反应性,缓解或解除支气管平滑肌痉挛,改善通气功能,达到治疗支气管哮喘的目的。

Observation of Clinical Efficacy on Asthma of Heat Pattern Treated with Xiebai Qinggan Decoction

Objective To observe the clinical efficacy on bronchial asthma of heat pattern treated with Xiebai Qinggan Decoction. Methods From July 2010 to December 2013,72 cases of bronchial asthma differentiated as heat pattern at attack stage were collected and randomized into two groups,36 cases in each one. The baseline treatment was given in the two groups. Additionally,Xiebai Qinggan Decoction was supplemented for oral administration in the treatment group and Aminophyline Tablets were prescribed for oral administration in the control group. The clinical efficacy was analyzed statistically in the two groups. The improvements in main symptoms and physical signs and changes in esonophils（ EOS） and leukocytes（ LEU） were compared before and after treatment. Results The difference was not significant in the total effective rate between the two groups（ P〈0. 05）. The efficacy on cough and expectoration in the treatment group was significantly higher than that in the control group（ P〈0. 05）. Compared with that before treatment,EOS was improved significantly after treatment in the two groups,but there was no significant difference between the groups（ P〈0. 05）. LEU count was reduced significantly in the treatment as compared with that before treatment（ P〈0. 05） and the improvement in the control group was not significant as compared with that before treatment（ P〈0. 05）. The adverse reaction was not discovered in the two groups. Conclusion Xiebai Qinggan Decoction relieves significantly cough and expectoration. By reducing the abnormal increase of EOS and LEO counts in the peripheral blood,the airway inflammation and bronchial hyperresponsiveness are alleviated,bronchial smooth muscle spasm is alleviated or released and ventilation function is improved so that bronchial asthma is treated effectively in the patients of asthma at acute attack stage.
