骨折髋支撑关节治疗股骨颈骨折的实验研究

摘要：目的 通过动物实验探讨一种新的非限制性支撑固定装置—骨折髋支撑关节对新鲜头下型股骨颈骨折的治疗效果。方法 采用骨龄成熟的杂交犬32只，术中造股骨颈头下型骨折并股骨头脱位的动物模型。实验组用手术植入骨折髋，坚强固定骨折端、减轻并替代股骨头负重。对照组复位后用两枚螺丝钉固定骨折端。结果24周实验组骨折线区形成规则骨小梁并出现髓细胞、股骨头轻度纤维化，对照组骨折线区仍为类骨组织，股骨头坏死并塌陷变形。48周实验组骨折线区骨小梁改建，髓细胞丰富，股骨头恢复正常。对照组骨折线区出现少量不规则骨小梁。股骨头坏死塌陷并吸收。 结论 骨折髋支撑关节能有效的治愈股骨颈骨折并能有效的防止股骨头坏死。

Two different method to treat basal neck fracture

Objective To observe the therapeutic efficacy of fracture hip support joint (FHSJ) implantation on the fresh femoral neck fracture. Methods Thirty-two skeletal mature hybrid dogs were equally divided into experimental group and control group at random. The animal model of femoral neck fracture was adopted by cutting the femoral neck and round ligament of femur. FHSJ was planted into the hips of the experimental group. On the contrary, two screws were used to fix the fracture of the control group. Results At 24 weeks, trabecular bone of the fracture area became regular and marrow cells came out in the experimental group, and there were only a few fibrosis of the femoral head. At the same time, osteoid tissue was still seen in the fracture area, and the avascular necrosis, collapse and deformity of femoral head appeared in the contrary group. At 48 weeks, bone trabecula tissue was rebuilded around the fracture line and plentiful medullocells appeared, and the femoral head of the experitmental group. There were only a few irregular trabecular bones in the fracture area and the height recuperated to normal appearance. Conclusion The plantation of FHSJ, which can not only improve the treat of the femoral neck fracture, but also prevent the avascular necrosis of the femoral head with the femoral neck fracture, is an available method to heal the basal neck fracture.
