心理护理在初发2型糖尿病胰岛素泵强化治疗中的作用

目的:探讨胰岛素泵强化治疗初发2型糖尿病时,心理护理对治疗效果的影响。方法:30例初发的2型糖尿病患者进行胰岛素泵强化治疗中,除采用饮食指导、运动指导、胰岛素泵护理、低血糖防治等方面的综合护理措施外,给予心理护理,观察其治疗中血糖达标时间、低血糖发生率、治疗后停药缓解率,并与采用常规综合护理的对照组比较。采用配对t检验和两个样本率的检验。结果:与常规护理组相比,加用心理护理组,患者配合治疗的依从性提高,血糖达标早[(5.6±1.4)d vs(6.7±1.8)d,P<0.01],撤泵后无药缓解率高(22/30 vs 28/30,P<0.05);低血糖发生率降低,但统计学无显著性(8/30 vs 14/30,P>0.05)结论:对初发2型糖尿病进行胰岛素泵强化治疗时,在采用常规综合护理措施外,给予心理护理,可以缩短达标时间,提高治疗效果。

Role of psychological nursing in newly-diagnosed type 2 diabetes patients under intensive insulin pump treatment

Objective ：To explore role of psychological nursing in newly-diagnosed type 2 diabetes patients under intensive in- sulin pump treatment. Methods：Thirty newly-diagnosed type 2 diabetes patients under the intensive insulin pump treatment were di- vided into routine nursing group and psychological nursing group, wherein the cases were all given the diet nursing, exercise care, in- sulin pump nursing, hypoglycemia nursing, besides that, those in psychological group were also given the psychological nursing. The time for controlling the blood glucose to the normal range, incidence rate of hypoglycemia, and withdrawal remission rate after the treat- ment were observed and compared by using paired t test and two sample rate. Results ： Compared with those of routine nursing group, the treatment compliance of the patients in psychological group was improved, the time for controlling the blood glucose to the normal range was shortened [ （5.6~1.4） days vs （6.7＋1.8 days）, P〈0.01], the withdrawal remission rate was higher （28/30 vs 22/30, P 〈0.05 ） , and the incidence rate of hypoglycemia was lower but the difference was not statistically significant （8/30 vs 14/30, P〉0. 05 ）. Conclusions ： For the newly-diagnosed type 2 diabetes patients under intensive insulin pump treatment, the psychological nursing based on the routine comprehensive nursing can shorten the control time and improve the treatment effects.
