老年抑郁症患者抑郁与甲状腺激素相关性的临床研究

目的探讨老年抑郁症患者抑郁与甲状腺激素相关性。方法采用自身对照研究方法,入院后,60例老年抑郁症合并甲状腺功能减退患者均给予舍曲林治疗,治疗时间为4周。治疗前及治疗后,采用汉密尔顿抑郁量表(HAMD)评定患者病情,采用酶联吸附试验(ELISA)检测血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)、血清游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)浓度,并分析HAMD与甲状腺激素的关系。结果治疗前,60例老年抑郁症患者HAMD评分为(31.35±4.17)分,T3、T4、FT3、FT4、TSH分别为(1.08±0.38)、(52.37.±3.42)、(2.96±0.43)、(7.93±0.58)、(5.37±0.46),治疗后,HAMD评分为(13.67±2.19)分,T3、T4、FT3、FT4、TSH分别为(2.11±0.71)、(56.15±5.28)、(4.67±0.38)、(12.59±0.66)、(3.19±0.64)。相关分析发现,HAMD与T4、FT4、TSH具有显著相关性(P<0.01)。结论抑郁与甲状腺激素具有相关性,精神科临床医师应综合治疗老年抑郁症。

The Clinical Study of the Correlation of between Depression and Thyroid Hormone of the Patients with Depression in old Age

Objective To explore the correlation of between depression and thyroid hormone of the patients with depression in old age.Methods Using self control study method, 60 patients with depression merging hypothyroidism in old age were all given Sertraline treatment ,after they were in hospital, the therapy time last 4weeks. Before and after therapy, the state of an illness of the patients were evaluated using HAMD, The concentration of T3, T4, FT3, FT4, TSH were detected using ELISA, and the relation of between HAMD and thyroid hormones was analyzed.Results Before therapy, the score of HAMD T3, T4, FT3, FT4, TSH of the parents with depression in old age were respectively（31.35＆#177;4.17）,（1.08＆#177;0.38）, （52.37.＆#177;3.42）,（2.96＆#177;0.43）,（7.93＆#177;0.58）,（5.37＆#177;0.46）,after therapy, the score of HAMD T3, T4, FT3, FT4, TSH of the parents with depression in old age were respectively （13.67＆#177;2.19）, （2.11＆#177;0.71）,（56.15＆#177;5.28）,（4.67＆#177;0.38）,（12.59＆#177;0.66）,（3.19＆#177;0.64）. Correlation analysis showed, there was significant correlation between HAMD and T3, T4, FT3, FT4, TSH（P〈0.01）.Conclusion There was correlation between depression and thyroid hormones, Psychiatric clinicians should cure depression in old age comprehensively.
