腹腔镜下闭孔疝的诊断与治疗体会

目的:探讨腹腔镜下闭孔疝的诊断和治疗。方法:回顾性分析应用腹腔镜诊断治疗的15例闭孔疝临床资料。结果:15例均在腹腔镜下手术成功,无中转开腹,平均手术时间55 min,5例小肠坏死行肠切除术,1例大网膜嵌顿行坏死部分切除术,术后1例发生肺部感染,1例伤口感染,余无并发症。肠功能恢复时间1～4.5 d,平均2.5 d;住院时间3～11 d,平均5.6 d;随访时间1.5～5年,平均2.2年。无肠瘘、复发、肠梗阻。结论:腹腔镜诊断治疗闭孔疝具有安全、可行、有效、微创的优势。

The Diagnosis and Treatment of Obturator Hernia Under the Laparoscope

Objective To explore the diagnosis and treatment of obturator hernia under the laparoscope.Methods 15 cases of obturator hernia were diagnosed and treated under the laparoscope.Results 15 cases of laparoscopic appendectomy were successfully performed and no case was transferred to opening appendectomy,the average operation time was 55 min.5 cases of small bowel necrosis required resection,1 case of omental incarceration necrotic part resection,1 patient with postoperative pulmonary infection,1 cases of wound infection,and the rest with no complications.The average time of intestinal function recovery was 2.5 day(1～4.5 d),hospital day was 5.6 d(3～11 d) and Follow-up for an average of 2.2 years(1.5～5 y).There is no intestinal fistula,recurrence and intestinal obstruction for average 6 monthes.Conclusion The laparoscopic diagnosis and treatment of obturator hernia has the advantages of security,feasibility,effectiveness and minor injury.
