五行辩证针刺治疗对卒中后抑郁患者生活质量的影响

目的：观察五行辩证针刺治疗对卒中后抑郁患者抑郁情绪及生活质量的影响。方法将150例卒中后抑郁患者按随机数字表法分为3组，每组50例，均予以神经内科常规治疗及康复训练，西药组予以盐酸帕罗西汀治疗，针刺组予以盐酸帕罗西汀联合常规针刺治疗，五行针刺组予以盐酸帕罗西汀联合五行辩证针刺治疗。治疗4周，观察3个月。于治疗前后采用抑郁自评量表及脑卒中患者生活质量量表评定抑郁情绪及生活质量。结果治疗4周末起3组抑郁自评量表评分均较治疗前显著降低(P <0.01)；五行针刺组显著低于西药组及针刺组(P <0.05或0.01)。治疗1周末起3组脑卒中患者生活质量量表评分均较治疗前显著升高(P <0.05或0.01)；治疗4周末五行针刺组显著高于西药组及针刺组(P <0.05或0.01)，治疗3个月末仍显著高于西药组(P <0.01)。结论抗抑郁剂联合五行辩证针刺治疗卒中后抑郁具有增效作用，有利于改善患者的抑郁情绪及生活质量。

Effects of the five elements syndrome differentiation acupunc-ture treatment on quality of life of post-stroke depression pa-tient

Objective To observe the effects of the five elements syndrome differ-entiation acupuncture treatment on quality of life of post-stroke depression (PSD) patient.Methods A total of 150 PSD patients were assigned to 3 groups of 50 ones each,they all received neurological routine treatment and rehabilitation training,Western medicine group (WG)was treated with paroxetine hydrochlo-ride,acupuncture group (AG)with paroxetine hydrochloride plus acupuncture, five elements acupuncture group (FG)with five paroxetine hydrochloride plus elements syndrome differen-tiation acupuncture treatment for 4 weeks,and observed for 3 months.Before and after treatment depres-sive emotions were assessed with the Self-rating Depression Scale (SDS)and qualities of life with the Qual-ity of Life Scale (QOL).Results Since the end of the 4 th week the SDS scores of 3 groups lowered more significantly compared with pretreatment (P <0.01)and that was significantly lower in FG than in WG and AG (P <0.05 or 0.01).Since the end of the 1 st week the QOL scores of 3 groups heightened more significantly compare with pretreatment (P <0.05 or 0.01);at the end of the 4 th week the QOL score was significantly higher in FG than in WG and AG (P <0.05 or 0.01),so was that than in WG at the end of 3 month treatment (P <0.01 ).Conclusion Antidepressants plus five elements syndrome dif-ferentiation acupuncture treatment have a synergi-sm in PSD and are beneficial to the improvement of patient’s depressive emotions and quality of life.
