三维适形放疗治疗局部进展期非小细胞肺癌疗效分析

目的：研究三维适形放射治疗局部进展期非小细胞肺癌（NSCLC）的疗效和毒副作用。方法：56例局部进展期非小细胞肺癌患者均采用三维适形放疗,先行计划靶区（PTV）照射45-50 Gy,后缩野肿瘤区（GTV）加量照射累积剂量DT61-67 Gy,均为1.8-2.0 Gy/次,5次/周。结果：完全缓解（CR）17例（30.4%）,部分缓解（PR）32例（57.1%）。1年生存率75.0%,2年生存率33.9%。毒副作用以放射性食管炎及放射性肺炎为主,但均可耐受。结论：三维适形放疗可提高局部进展期非小细胞肺癌患者局控率及1、2年生存率,减轻放疗的毒副作用。

Effect analyzed on three dimensional conformal radiationtherapy for local progressive non-small lung cancer

Objective: To evaluate the effect and toxicity of 3-dimensional conformal radiation therapy(3DCRT) in treating local progressive non-small lung cancer(NSCLC).Methods: Fifty-six patients with local progressive non-small lung cancer were radiated by 3DCRT.They first receive 45~50 Gy to PTV,then boosted to GTV to a total dose DT 61~67 Gy.They all were executed at 1.8~2.0 Gy/fraction,5 fraction/week.Result: There were 17 cases of CR(30.4%) and 30 cases of PR(57.1%).The overall 1,2 year survive rates were 75.0% and 33.9% respectively.Serious toxicity was found to be the radiation esophagitis and radiation pulmonitis,but tolerable.Conclusion: 3DCRT could enhance the local control rate and 1,2 year survive rate of local progressive NSCLC and reduce serious radiation toxicity.
