子宫瘢痕对门诊宫腔镜术患者疼痛的影响

目的探讨子宫瘢痕对门诊宫腔镜术患者疼痛的影响。方法采用前瞻性单盲随机对照研究的方法,纳入140例行门诊宫腔镜检查的育龄期女性,将患者分为两组:A组为子宫瘢痕组(n=70),B组为无子宫瘢痕组(n=70)。所有患者均未尝试阴道分娩。门诊宫腔镜检查时不麻醉或镇痛。使用100.0 mm视觉模拟量表评估患者手术时和术后疼痛程度,并评估手术顺利完成程度。结果手术时或术后患瘢痕性子宫与无瘢痕子宫患者疼痛评分差异无统计学意义。子宫瘢痕组中仅1例患者中止手术,该患者有手术部位感染病史,可能导致轻微瘢痕。剖腹产瘢痕与肌瘤切除术瘢痕患者间的疼痛评分差异无统计学意义。在1、2、3或4次例剖腹产术患者间疼痛评分差异无统计学意义。结论在使用3.8 mm宫腔镜行诊断性门诊宫腔镜检查时,非复杂子宫瘢痕对检查时或之后的疼痛没有影响。

Impact of uterine scar on pain experiencing during outpatient hysteroscopy

Abstract: Objective To study the impact of uterine scar on pain experiencing during outpatient hysteroscopy. Methods A prospective blinded comparative study were performed in Outpatient hysteroscopy clinic. We included 140 women in the childbearing period attending an outpatient hysteroscopy clinic. Patients were divided into 2 groups. Group A included patients with previous uterine scar (n = 70) and Group B included those with unscarred uterus (n = 70). None of the patients had a previous attempt of a vaginal delivery. Diagnostic outpatient hysteroscopy without the use of anesthesia or analgesia. We assessed pain experiencing during and immediately after the procedure using a 100.0 mm visual analog scale. We also evaluated the successful completion of the procedure. Results There were no statistically significant differences in the pain scores between patients with scarred uterus and those with unscarred uterus during or immediately after the procedure. The procedure was aborted in only 1 case in the scarred uterus group. This patient had a history of surgical site infection, which may denote a weak scar. There were no statistically significant differences in pain scores between patients with cesarean scar and those with myomectomy scar. No statistically significant differences in pain scores were found between patients with 1, 2, 3, or 4 cesarean deliveries. Conclusion Uncomplicated uterine scars do not have an impact on pain experienced during or immediately after diagnostic outpatient hysteroscopy using a 3.8 mm hysteroscope.
