瑞芬太尼及丙泊酚静脉复合麻醉在妇科腹腔镜手术中的应用

目的观察瑞芬太尼、丙泊酚静脉复合麻醉在妇科腹腔镜手术中的麻醉效果。方法选择200例ASAⅠ～Ⅱ级、心功能Ⅰ～Ⅱ级的妇科患者,麻醉诱导：咪达唑仑2mg、芬太尼0.1mg、丙泊酚1.0～1.5mg/kg、琥珀胆碱注射液100mg静注,1min后显露声门插入气管导管;麻醉维持：5%葡萄糖注射液250mL＋丙泊酚400mg＋瑞芬太尼1mg,静脉点滴,使用顺式阿曲库铵4～6mg,维持术中肌肉松弛。结果所有患者术中麻醉平稳,Bp、HR、ECG、SPO2、PETCO2监测均正常。停止静滴麻醉复合液,减浅麻醉,拔除气管导管,患者迅速清醒。结论瑞芬太尼及丙泊酚静脉复合麻醉可控性强,且术后苏醒迅速,降低了因麻醉过深所带来的各种不良反应和副作用,减轻了患者的痛苦,是妇科腹腔镜手术一种理想的麻醉方法。

The application of remifentanil-propofol compound intravenous anesthesia in gynecological laparoscopic surgery

Objective To observe the effects of remifentanil-propofol compound intravenous anesthesia in gynecological laparoscopic surgery.Methods Two hundred gynecological patients with ASA Ⅰ-Ⅱ,cardiac function Ⅰ-Ⅱ were sellected.Anesthesia induction:midazolam 2 mg,fentanil 0.1 mg,propofol 1.0-1.5 mg/kg,succinylcholine 100 mg intravenous injection,1 min later reveal the glottis,insert into the tracheal tube.Anesthesia maintenance:5% glucose injection 250 ml,propofol 400 mg plus remifentanil 1 mg,intravenous drip,cisatracurium 4-6 mg were used to maintain muscle relaxation.Results All patients underwent anesthesia smoothly,Bp,HR,ECG,SPO2 and PETCO2 monitoring are normal.The patients quickly awake after stop dripping intravenous anesthesia compound liquid and tracheal extubation.Conclusion Remifentanil-propofol anesthesia is controllable and has rapid postoperative recovery.It reduces the various adverse reactions and side effects due to too deep anesthesia,and lessens the patients’ pain.It is particularly suitable for laparoscopic surgery anesthesia.
