ABCD2评分联合DSA在短暂性脑缺血发作转归中的临床研究

目的探讨ABCD2评分法联合数字减影血管造影（DSA）检查在短暂性脑缺血发作（TIA）后7d及30d内脑卒中发生风险的预测价值。方法用ABCD2评分对82例TIA患者进行评分，观察7d及30d内脑梗死的发生率，并根据DSA结果分为血管狭窄≥50％组和血管狭窄〈50％组。结果ABCD2评分0～3分TIA1周内脑梗死发生率为4．34％，4～5分脑梗死发生率为16．22％，6～7分者为31．82％。30d时分别为13．04％，18．91％，40．91％。血管狭窄≥50％组7d内脑梗死发生率为46．15％，30d内高达61．54％；血管〈50％组7d内脑梗死发生率为3．57％，30d内为5．36％，两组比较差异有统计学意义。结论ABCD2评分联合DSA检查是预测TIA7d及30d脑梗死发生风险的有效方法，值得临床推广应用。

The relationship between ABCD2 score and the assessment of transient ischemic attack's prognosis

Objective To research the clinical value of the ABCD2 score uniting with DSA in predicting cerebral infarction at the 7th day and the thirty day after transient ischemic attack. Methods Eighty two cases of TIA patients were evaluated according to ABCD2 criteria. The occurrences of cerebral infarction were observed in 7th and the thirty days, all patients were divided into two groups according to cerebrovascular stenosis ﹥50% or not. Results The patients scored 0-3, and the occurrence of cerebral infarction was 4.34%, the patients scored 4-5 was 16.22%. The patients scored 6-7 and the occurrence was 31.82% at the 7th day, 13.04%,18.91%,40.91% at the thirty day. The occurrence of cerebral infarction of group stenosis ≥50% in 7 days and 30 d(46.15%, 61.54%) had significant difference with that of group stenosis < 50%(3.57%, 5.36%). Conclusion ABCD2 criteria score uniting with DSA is effective to predict the occurrence of cerebral infarction in 7 and 30 d in TIA, it should be widely applied to clinic。
