经直肠腔内彩色多普勒超声检查对前列腺疾病诊断的临床价值

目的：探讨经直肠腔内彩色多普勒超声检查前列腺疾病的临床价值。方法：对80例54～91岁男性经直肠指检（DRE）检出前列腺增大患者应用经腹壁及经直肠腔内超声（TRUS）检查，同时进行前列腺特异抗原（PSA）检测分析，并全部经尿道电气化切除（TUVP）或经腹手术病理证实。结果：前列腺癌19例，单纯前列腺增生以及伴有炎症、钙化、囊肿等共61例。经直肠超声检查前列腺癌的准确度为84％，而经腹超声检查与PSA检测前列腺癌的准确度分别为71％与68％，经直肠超声比经腹超声敏感性更高（P〈0.01），比PSA特异性更高（P〈0．05），差异有显著性意义。结论：经直肠检查前列腺，图像清晰，测量准确，分辨力佳，是诊断前列腺疾患的最佳途径和方法之一，结合血流特征和PSA检测可提高对前列腺癌的诊断率。

Clinical value of transrectal color doppler ultrasound for prostatic disease

Objective: To evaluate the clinical value of transrectal color Doppler ultrasound in diagnosing prostate disease.Methods: A total of 80 patients with prostatic hypertrophy due to digital rectal examination(DRE) underwent abdominal ultrasound and transrectal ultrasound(TRUS) examination,the results were compared with the pathology and PSA. Results: Among 80 patients 19 cases of prostatic cancer(PC) were confirmed by transurethral electrovaporization of the prostate(TUVP) and pathology,the other 61 had benign prostate hypertrophy(BPH) associated with prostatitis,calcification or cyst.The degree of accuracy in diagnosing prostate cancer with TRUS was 84%,while transabdominal ultrasound was 71% and PSA was 68%.TRUS was more sensitive to PC than transabdominal ultrasound(P<0.01) and more specific than PSA(P<0.05).There was significant difference of detecting rate between them. Conclusion: TRUS was one of the best methods of diagnosis for prostate disease owing to its advantages of clear images,accurate measurement and good sharpness.TRUS combined with color Doppler and PSA can improve the diagnostic accuracy of PC.
