腰大池持续引流治疗重型颅脑损伤并蛛网膜下腔出血的临床分析

目的观察腰大池持续引流治疗重型颅脑损伤并蛛网膜下腔出血（tSAH）的临床效果。方法选择2009年1月～2013年12月150例重型颅脑损伤并蛛网膜下腔出血患者作为研究对象，将其随机分为两组，对照组（n=75）行腰椎穿刺放液，观察组（n=75）行腰大池脑脊液持续引流；评估并对比两组患者的治疗效果。结果观察组患者在蛛网膜下腔积血清除时间、临床预后以及脑梗死、脑积水、癫痫等并发症发生率方面均要优于对照组（P〈0．05）。结论腰大池持续引流不仅可以有效清除蛛网膜下腔积血，而且操作简便、并发症少、预后良好，体现了较高的临床价值。

Clinical analysis of continuous lumbar cistern drainage in treatment of severe craniocerebral injury concurrent with subarachnoid hemorrhage

Objective To observe the clinical effect of continuous lumbar cistern drainage in the treatment of severe craniocerebral injury concurrent with suharachnoid hemorrhage（tSAH）. Methods 150 patients with severe craniocerebral injury concurrent with subarachnoid hemorrhage from January 2009 to December 2013 were selected as the study subjects and randomly divided into 2 groups. The control group（n=75） received lumbar paracentesis and the observation group（n=75） received continuous lumbar cistern drainage. The treatment effects of the 2 groups of patients were evaluated and compared. Results The observation group was better than the control group in the subarachnoid hematocele clearance time, clinical prognosis and incidences of cerebral infarction, hydrocephalus, epilepsy and other complications（P 〈 0.05）. Conclusion Continuous lumbar cistern drainage not only can effectively clear subarachnoid hematocele, but also is easy to operate, causes few complications and enables good prognosis, reflecting high clinical value.
