负压封闭引流技术在创伤性小腿骨筋膜室综合征中的应用

摘    要:目的探讨负压封闭引流技术(VSD)在创伤性小腿骨筋膜室综合征(OCS)切开减压中的临床应用。方法 2010年3月~2011年12月采用VSD闭合的16例小腿OCS患者,其中男性10例,女性6例;年龄17~62岁,平均年龄34.6岁。共计26处减张切口。回顾性研究根据拆除VSD后创面情况选择闭合切口时机。结果所有患者患肢减压有效,术后7 d拆除VSD。20处创面拆除VSD后予以直接缝合闭合;6处创面通过更换VSD,再次拆除VSD后予以完全缝合闭合。术后无切口感染不愈合情况发生。结论VSD技术在小腿OCS早期切开减压治疗中能快速覆盖创面,降低感染概率,促进肉芽组织生长,利于创面修复。

Abstract:Objective To investigate the application of vacuum sealing drainage (VSD) for the incision decompressions in osteo- fascial compartment syndrome(OCS) of leg. Methods The 16 patients with OCS of leg from March 2010 to December 2011 were enrolled, male 10, female 6, aged 17 - 62 years old, mean age 34.6 years old, which with 26 decompressed incisions. The 16 leg- injured OCS patients were treated by incision decompressions with VSD, and the closed incisions time was based on VSD- removed surface of wound. Results Decompression of all the leg was effective. VSD was removed after 7-day, 20 wound was closed by direct suture, 6 wound was closed after second VSD therapy. No wound infection occurred in all patients. Conclusion It is demonstrated that VSD could cover the wound rapidly after incision decompressions in OCS of leg, prevent infection, promote capillary and granulation tissue growing and is favorable for wound repairing.
