孕通汤联合宫腹腔镜治疗输卵管性不孕症的分析

目的分析孕通汤联合宫腹腔镜手术治疗输卵管性不孕症的临床价值。方法选择2015年5月至2017年5月河北省沧州中西医结合医院收治的204例子宫输卵管造影检查确诊为输卵管性不孕症的患者作为研究对象。均经腹腔镜手术证实,采用宫腹腔镜治疗。按随机数字表法分为治疗组(n=102)和对照组(n=102)。对照组采用宫腹腔镜手术治疗,治疗组在对照组基础上术后予以孕通汤治疗。电话随访两组1年内的妊娠情况。结果随访期间治疗组和对照组分别退出1例,失访1例;治疗组术后妊娠率为68.0%,高于对照组的51.0%,差异具有统计学意义(P<0.05)。孕通汤对妊娠率的影响随着输卵管病变程度的加重而减小,输卵管Ⅰ级病变妊娠率91.3%,输卵管Ⅱ级病变妊娠率65.5%,输卵管Ⅲ级病变妊娠率28.0%。结论孕通汤联合宫腹腔镜手术治疗输卵管性不孕症疗效确切,但在输卵管Ⅲ级病变患者中疗效不明显。

Analysis on Yuntong Decoction combined with hysterolaparoscopy in the treatment of tubal infertility

Objective To analyse the clinical value of Yuntong Decoction combined with hysterolaparoscopy in the treatment of tubal infertility.Methods A total of 204 patients diagnosed as tubal infertility by hystero-salpingography in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine of Hebei Province from May 2015 to May 2017 were selected as the research subjects.All patients were confirmed by laparoscopic surgery and treated by uterine laparoscopy.According to random number table method,they were divided into treatment group(n=102)and control group(n=102).The control group was treated with uterine laparoscopic surgery,and the treatment group was combined with Yuantong Decoction.Pregnancy in the two groups was followed up by telephone within 1 year.Results During the follow-up period,the observation group and the control group withdrew 1 case and lost 1 case respectively.The pregnancy rate in the treatment group was 68%,significantly higher than 51.0%of the control group.The effect of Yuntong Decoction on pregnancy rate decreased with the aggravation of fallopian tube lesions,the pregnancy rate was 91.3%in gradeⅠ,65.5%in gradeⅡand 28.0%in gradeⅢtubal lesions.Conclusions Yongtong Decoction combined with hysterolaparoscopy is effective in the treatment of tubal infertility.But in the patients with gradeⅢfallopian tube lesions,the curative effect is not obvious.
