剖宫产手术患者围手术期抗菌药物应用的前瞻性调查

目的了解剖宫产患者围手术期抗菌药物的应用现状,以进一步指导临床合理用药。方法采用前瞻性调查方法,对2007年6月1日~12月31日产科所有剖宫产手术患者1667例进行了调查,经统计学处理,了解剖宫产手术患者围手术期抗菌药物的应用情况。结果医院剖宫产手术患者平均住院日为7.5 d,平均应用抗菌药物3.6 d;抗菌药物应用率100.0%;择期剖宫产全部是术后给药,药物的联合使用率为11.7%;术前应用抗菌药物前3位排序为大环内酯类、头孢一代、头孢二代,术后依次为头孢一代、大环内酯类、头孢二代;术后大部分抗菌药物利用指数(DUI)1;术后切口感染率0.1%。结论医院剖宫产手术患者抗菌药物应用趋于合理,预防性应用抗菌药物时间尚需要进一步减少。

Prophylactic Antibiotics Usage During Cesarean Perioperative Period: A Prospective Survey

OBJECTIVE To investigate and analyze the application of prophylactic antibiotics during cesarean perioperative period in hospital.METHODS Totally 1667 cases under cesarean section were investigated prospectively from Jun to Dec in 2007.RESULTS The mean time staying in hospital was 7.5 days.The mean antibiotics using time was 3.6 days.The rate of antibiotics usage was 100%.All of selective operation cases applied antibiotics after operation procedure.The rate of antibiotics combination using was 11.7%.The first three kinds of antibiotics preoperation includes macrolides, the first generation of cephalosporin and the second generation of cephalosporin.The first three kinds of antibiotics postoperation were the first generation of cephalosporin, macrolides and the second generation of cephalosporin.Most of drug use indexs (DUI) were <1.0.The surgical site infection (SSI) rate was 0.1%.CONCLUSIONS The application of prophylactic antibiotics during cesarean perioperative period in our hospital is relatively reasonable.The prophylactic antibiotic usage time still should be reduced further.
