地佐辛超前镇痛对腹腔镜胆囊切除术后镇痛效果的影响

目的探讨地佐辛超前镇痛对腹腔镜胆囊切除术后镇痛效果的影响。方法 40例择期行腹腔镜胆囊切除术患者（ASA分级Ⅰ、Ⅱ级）,随机分成超前镇痛组和术后镇痛组。超前镇痛组在全麻诱导时静脉缓慢推注地佐辛0.1 mg/kg,术后镇痛组在手术结束时静脉缓慢推注地佐辛0.1 mg/kg。手术结束后3、6、12、24 h对患者术后的疼痛程度采用视觉模拟评分法（VAS）进行评分并记录术后止痛药的需求情况及术后不良反应。结果超前镇痛组术后3、6、12、24h VAS评分明显低于术后镇痛组。超前镇痛组没有患者术后需要止痛药,术后镇痛组却有4例。两组恶心、呕吐发生率差异无统计学意义。结论地佐辛术前应用在腹腔镜胆囊切除术中能起到超前镇痛作用,可安全而有效地减轻术后疼痛。

The preemptive analgesic effect of dezocine injection in patients undergoing laparoscopic cholecystectomy

Objective To study the preemptive analgesic effect of dezocine injection in patients undergoing laparoscopic cholecystectomy.Methods Forty patients undergoing laparoscopic cholecystectomy were equally randomized into two groups.The patients in the preemptive analgesic group were intravenously injected with dezocine 0.1 mg/kg before intubation.The patients in the postoperative analgesic group were intravenously given dezocine 0.1 mg/kg at the end of the surgery.The analgesic effect was evaluated by visual analog scale（VAS） score at 3,6,12 and 24 h after surgery,and the side effect and the amount of analgesic drugs used postoperatively were recorded.Results VAS scores of the preemptive analgesic group were significantly higher than those of the postoperative analgesic group at 3,6,12 and 24 h after surgery（P0.05）.There was no patient who needed analgesic drugs in the preemptive analgesic group,while there were four patients who needed analgesic drugs in the postoperative analgesic group.The side effects of the two groups were similar.Conclusion Preemptive analgesia with dezocine for postoperative analgesia is effective and safe in patients undergoing laparoscopic cholecystectomy.
