面中部骨折的手术治疗

目的探讨面中部骨折手术治疗的不同路径和固定方法。方法32例面中部骨折患者采用口内龈颊沟切口,配合眶外缘、眶下缘及头皮冠状切口,骨折复位后用小(微)型钛板固定。结果32例患者伤口全部I期愈合。术后3个月,患者开口度≥25mm,咬合关系良好,X线片显示骨断端对位良好,9例面部畸形患者除1例颊部轻度凹陷、2例颧部略显突出外,基本恢复。7例眶下区麻木患者仍有2例感觉麻木,但范围明显缩小。2例眼球下沉、内陷伴复视患者基本恢复正常。结论面中部骨折手术疗效可靠,应根据不同类型和部位的骨折,选择不同的切口和固位方法。

Surgical Treatment of Midface Fracture

Objective To investigate different ways and fix methods of surgical operation of midface fracture.Methods The ditch of gum and check incision,the exterior and bottom of eye socket incision,and the skin of head coronals incision were made.After the relocation of bone fracture,it was then fixed with titanium mini plate.Thirty-two midface fracture cases were analyzed.The relationship between the fracture line,the mouth opening and malocclusion,the abnormal face and complications were examined by X-ray in 1 week and 3 month after operation.Results The incisions of thirty-two cases were healed in stage I.The mouth-opening of all the cases were over 25 mm.All the cases had satisfied occlusion.X-ray examination showed the fracture lines were good.In 9 cases with abnormal face,1 case had slight sunken cheekbone,while 2 cases had slight prominent cheekbone.2 cases still had slight numb feeling and 7 cases with infraorbit numbness.2 cases had sunken eyeball,but the diplopia of the patients were basically recovered.Conclusion The surgical operation on midface fracture is reliable and the choice operation method should depend on the type of bone fracture.
