胆总管探查术后胆总管Ⅰ期缝合32例临床分析

目的探讨胆总管探查术后胆总管Ⅰ期缝合的适应证及可行性。方法严格选取32例有病变的胆总管切开探查,取尽结石,术中纤维胆管镜及经胆囊管胆管造影检查肝内外胆管无残留结石,胆总管下端通畅,用无创线Ⅰ期缝合胆总管。结果术后1例患者少量漏胆汁,进行保守治疗痊愈,其余31例患者无任何并发症发生。结论胆总管探查术术中经胆管镜及胆管造影检查确认无残留结石、乳头开闭良好、胆管远端通畅者,行胆总管Ⅰ期缝合安全、有效。

Clinical Analysis of Primary Suture After Common Bile Duct Exploration in 32 Patients

Objective To investigate the feasibility and surgical indications of primary suture after exploration of common bile duct.Methods To select 32 cases after exploration of common bile duct strictly and clean all the stones. Fibercholedochoscope and X - ray examination of the bile ducts make sure that there are not residual stones and the end of common bile duct is not obstructed. Then it is sewed up primarily.Results 1 case with leaked bile after operation recovered by receiving conservative therapy and other cases had no complications. Conclusion After X - ray examination of the bile ducts combined with fibercholedochoscope for exploration of common bile duct make sure that common bile duct has not residual stone and unobstructed, primary suture of the common bile duct is safe and reliable.
