右心感染性心内膜炎的诊断及治疗（附八例报告）

目的：研究右心感染性心内膜炎（ＲＳＩＥ）的诊断和治疗方式,以及外科治疗的手术指征和时机。方法：回顾分析８例ＲＳＩＥ的临床表现,诊断和治疗情况。结果：７例符合Ｒｏｂｂｉｎｓ提出的临床诊断标准,１例在手术中诊断,１例内科治疗痊愈,７例合并基础心脏病者外科治疗痊愈,随访５个月～３年无复发。结论：确诊后应联合应用广谱抗生素治疗。对于内科治疗无主合并基础心脏值得应早期手术,手术时机分别选择在发病后２－４周和

The Diagnosis and Treatment of Right-sided Infective Endocarditis:A Report of 8 Cases

Objective: Studying the diagnostic criteria, therapeutic ways, surgical indication and timing for right-sided infective endocarditis(RSIE). Method: The clinical manifestation, diagnostic criteria and management of 8 patients with RSIE were analysed. Results:7 cases of them were agreed with clinical diagnostic criteria by Robbins, 1 case was diagnosed during operation. 7 patients with underlying heart disease were cured by surgery, 1 patient was cured with medication. All patients had full recovery after treatment, no recurrence after follow-up for 5 months to 3 years. Conclusion: High-dose and combined antibiotics should be used to treat RSIE patients. If medication was ineffective and the patient was accompanied with underlying heart disease, surgical operation should be performed early. It is better for patients to have cardiac surgery 1 to 4 weeks after a diagnosis is made. Cutting vegetations and valvuloplasty were mandatory.
