摘    要:目的 探讨血清中脂蛋白、载脂蛋白水平与冠心病(CHD)的关联.方法 以健康体检30例为对照组,48例高血压患者和51例CHD患者作为研究对象.分别检测血清中甘油三酯(TG)、胆固醇(TC)、高密度脂蛋白(HDL-c)、低密度脂蛋白(LDL-c)及ApoB、ApoA-Ⅰ、LP (a)的水平.结果 CHD组与对照组和高血压组比较,HDL-c、ApoA-Ⅰ、ApoB、ApoB/ApoA-Ⅰ差别均有统计学意义(均P<0.05),Logistic回归分析提示,HDL-c、ApoA-Ⅰ、ApoB、ApoB/ApoA-Ⅰ、LPa均对CHD有显著影响,且HDL-C、LP(a)是CHD独立危险因素;CHD与ApoB、Lp(a)及ApoB/ApoA-Ⅰ正相关,与HDL-c及ApoA-Ⅰ负相关.结论 共同检测和分析HDL-c、ApoB、ApoB/ApoA-Ⅰ与Lp(a)水平对冠心病的预测有一定价值.

Abstract:Objective To explore the association of serum lipoprotein and apolipoprotein levels with the coronary artery disease. Methods 48 patients with hypertension and 51 with coronary heart disease(CHD) were recruited for the present research. 30 healthy people were set up as control. The level of TG,TC,HDL-c ,LDL-c and ApoB,ApoA-I, LP (a) were detected. Results There were significant differences of the level of HDL-c,ApoA-I,ApoB,andApoB/ApoA-I(P~ 0.05)between the groups of CHD and hypertension. Among lipid indice, HDL-c,ApoA-I,ApoB,ApoB/ApoA-I,and LPa had a significant effect on CHD. HDL-c and LP(a) were independent risk factors for CHD by using Logistic stepwise re- gression analysis. CHD was positively correlated with ApoB,Lp (a) and ApoB/ApoA-I but negatively with HDL-c and ApoA-I. Conclusions HDL-c,ApoB,ApoB/ApoA-I and Lp(a) are helpful with evaluation of the risk factors of CHD.
