原发性胃淋巴瘤的多层螺旋CT诊断价值

目的探讨原发性胃淋巴瘤的多层螺旋CT表现特征及诊断价值。方法对11例经病理证实的原发性胃淋巴瘤患者的多层螺旋CT及临床资料进行回顾分析。结果 11例均为B细胞起源非霍奇金淋巴瘤。原发性胃淋巴瘤按CT表现分为弥漫增厚型、节段增厚型、局限增厚型及混合型,CT表现特征主要有：①多灶分布和广泛、显著胃壁增厚;②病变沿长轴侵犯有一定柔软度,胃腔无明显变形;③病变区胃浆膜面多清楚,胃周脂肪无明显受侵;④胃粘膜广泛粗大伴多发的浅溃疡。结论原发性胃淋巴瘤的多层螺旋CT表现特征对诊断和鉴别诊断有一定价值。

Discussing the application of multi-slice CT in diagnosing primary gastric lymphoma

Objective To discuss the primary gastric lymphoma′s manifestation characteristic of multi-slicel CT and its diagnosing value.Methods The diagnosing reports of multi-slice CT and clinic data of 11 cases of patho-verified primary gastric lymphoma were retrospectively analyzed.Results All the cases were B-cell originating Non-Hodgkin′s Lymphoma.According to the CT manifestation,these cases can be divided into four types as diffuse thickening,segmental thickening,regional thickening and mixed type.The characteristic of CT manifestations was as follows：①Mulifocality distribution with extensive and significant thickening gastric wall;②Smooth pathological changes invaded along prolate axis without evident deformation in gastral cavity;③Clear serosa surface in pathological area without significant invasion in perigastric fat;④Mucosa enlargement with multiple exulceratio simplex.Conclusion Multi-slice CT has significant value in diagnosis differential diagnosis of primary gastric lymphoma.
