盐酸环喷脱酯、复方托品卡胺和阿托品对儿童青少年瞳孔及屈光度影响的对比研究

目的:比较3种睫状肌麻痹剂对儿童青少年瞳孔大小和屈光度的作用。方法:随机将223例屈光不正患者分为两组,每组按年龄与屈光状态分成4个小组。两组先分别用0.5%复方托品卡胺和1%盐酸环喷脱酯眼水散瞳,瞳孔恢复后均以阿托品散瞳3天,每种药物散瞳后用直尺测量瞳孔直径并用电脑验光仪进行客观验光。结果:除12岁以上近视组,3种药物对各年龄组瞳孔大小的作用差异均有统计学意义(P<0.05),各年龄组中阿托品的睫状肌麻痹效果强于其他两种药物(P<0.05),除12岁以下近视组外(P<0.05),盐酸环喷脱酯与复方托品卡胺睫状肌麻痹效果无显著性差异。结论:扩瞳方面,0.5%复方托品卡胺眼水>1%阿托品凝胶>1%盐酸环喷脱酯眼水;睫状肌麻痹方面,阿托品强于复方托品卡胺和盐酸环喷脱酯,而后两者的睫状肌麻痹效果基本相当。对于首次已用过阿托品验光且持续佩戴眼镜患者,复诊时可考虑用复方脱品卡胺或盐酸环喷脱酯散瞳验光。
