参芎葡萄糖注射液对缺血再灌注损伤心肌的保护作用

摘    要:目的探讨参芎葡萄糖注射液对小鼠急性心肌缺血再灌注(I/R)损伤的保护作用及可能的分子机制。方法建立急性心肌缺血再灌注(Ischemia-Reperfusion,I/R)损伤的小鼠模型,将野生型小鼠(C57BL/6)随机分为3组,假手术组(Sham),缺血再灌注+生理盐水组(Vehicle),缺血再灌注+参芎葡萄糖注射液预处理组(20mL/kg,SL),术前2小时给予腹腔注射生理盐水或参芎葡萄糖注射液,于缺血再灌注后不同时间点,通过心脏超声观察各组小鼠心功能的变化,心肌组织TTC染色确定心梗面积大小,脱氧核糖核苷酸末端转移酶法(TUNEL)检测凋亡心肌,蛋白免疫印迹法(WesternBlot)及酶联免疫吸附法(ELISA)分别检测心肌组织以及血清炎症因子肿瘤坏死因子(TNF-α)表达水平变化。结果I/R手术48小时后,与假手术组相比,左室射血分数(LVEF)明显降低(P〈0.01)。参芎葡萄糖注射液预处理明显改善I/R组的LVEF,并降低了心肌梗死面积,抑制I/R引起的心肌细胞凋亡和组织炎症反应。进一步的研究表明,参芎葡萄糖注射液预处理明显抑制了I/R后心肌组织以及血清中TNF-α表达水平升高(P〈0.01)。结论参芎葡萄糖注射液预处理通过抑制TNF-α表达水平发挥对缺血再灌注(I/R)损伤心肌的保护作用。

Abstract:Objective To investigate the cardio-protective effect of Salvia miltiorrhiza Ligustrazine Hydrochloride and Glucose injection (SL) induced by ischemic pre-conditioning (IPC) following myocardial ischemia-reperfusion (I/R) injury in mice.Methods I/R injury model was obtained by ligating the proximal left anterior descending coronary artery 30 min followed by different hours reperfusion.Wild-type C57BL/6 mice were randomly divided into 3 groups and separately subjected to IPC treatments:Sham control group;I/R+Vehicle control group;I/R+SL group (20 mL/kg);The mice were treated with the SL by peritoneal injection 2 hours before the surgery.Cardiac function was detected by echocardiography,TTC staining was performed to evaluate the area of myocardial infarction,The number of apoptotic cardiomyocyte was detected by TUNEL assay,TNF-α levels in serum and myocardium was measured by ELISA and Western Blot,respectively.Results Cardiac function of mice in SL group were greatly improved at 48 hours after I/R surgery (P<0.01).Further research indicated that SL pretreatment significantly reduced infarct size,inhibited apoptosis,attenuated inflammation response in myocardium,and decreased TNF-α level in serum and myocardium,compared with the vehicle group (P<0.01).Conclusion "Salvia miltiorrhiza Ligustrazine Hydrochloride and Glucose injection" exerts cardio-protective effects on ischemia-reperfusion injury by inhibiting TNF-α level.
