经尿道前列腺电切术300例临床分析

目的对经尿道前列腺电切术（TURP）疗效影响因素的再认识，提高TURP质量。方法回顾性分析熟练掌握技术后300例良性前列腺增生（BPH）患者TURP资料。年龄52-92岁，平均64．8岁。术前B超测定前列腺体积16．3-210．4ml·s^-1，平均46．2ml。最大尿流率（Qmax）平均8．4ml·s^-1，平均尿流率（Qave）平均4．3ml·s^-1，剩余尿量（RU）平均75ml。IPSS评分平均23分。结果300例患者手术效果满意，切除前腺组织平均31．8g，电切时间平均51min，术中输血1例，无因穿孔或无法控制的出血需改行开放手术的病例，发生经尿道电切综合征（TURS）先兆2例，无TURS发生。255例（85％）患者随访1—32个月，Qmax平均增至16，9ml·s^-1，Qmax平均增至10．8ml·s^-1，Ru平均降至15ml。IPSS平均8分。各项指标与术前相比，差异均有统计学意义（P〈0．01）。结论术前、术中、术后充分认识TURP疗效影响因素，可进一步提高TURP质量。

Recognition of factors influencing therapeutic efficacy of transurethral resection of the prostate:a report of 300 cases
