床边主动脉球囊反搏联合机械通气治疗老年急性心肌梗死合并心源性休克患者35例的临床观察

目的探讨床边主动脉球囊反搏联合机械通气治疗老年急性心肌梗死并心源性休克的临床疗效。方法 35例老年急性心肌梗死（AMI）合并心源性休克患者在床边行主动脉内球囊反搏（IABP）合并给予机械通气（MV）治疗。观察心源性休克纠正时间、MV撤机时间、IABP撤机时间、死亡及并发症发生情况,并比较治疗前后血气分析指标。结果本组35例患者死亡15例,病死率为42.85%;治疗前后平均动脉压（MAP）、心率（HR）、动脉血氧分压（PaO2）、动脉血二氧化碳分压（PaCO2）均有显著改善,差异有高度统计学意义（P〈0.01）。结论床边IABP联合MV治疗老年AMI合并心源性休克疗效确切,应尽早使用。

Clinical observation of bedside IABP combined with MV in treatment of 35 elderly AMI patients combined with cardiogenic shock

Objective To investigate the clinical effect of bedside intra-aortic balloon pump(IABP) combined with mechanical ventilation(MV) in the treatment of 35 elderly AMI patients combined with cardiogenic shock.Methods 35 cases of elderly AMI patients combined with cardiogenic shock were treated with bedside IABP and MV at the same time.The correct time of cardiogenic shock,MV and IABP removing time,incidence of death and complications,and blood gas analysis index before and after treatment were observed.Results Among 35 patients,15 cases were dead,the mortality rate was 42.85%.The MAP,HR,PaO2 and PaCO2 of patients after treatment were significantly improved than those of before treatment,the differences were statistically significant(P < 0.01).Conclusion The efficacy of bedside IABP combined with MV in treatment of elderly AMI patients combined with cardiogenic shock is exactly,which should be used as soon as possible.
