慢性肝病肝脏储备功能与肝纤维化的相关性分析

目的探讨慢性肝病患者肝脏储备功能与肝纤维化的相关性,以期为临床提供参考。方法回顾性分析本院收治的48例慢性肝病患者的临床资料,将患者分为慢性乙型肝炎组和乙型肝炎肝硬化代偿期组,对两组患者进行吲哚氰绿（ICG）排泄试验、肝脏硬度检测、肝脏组织纤维化定性分析和终末期肝病模型（MELD）评分。结果随着肝硬化程度的增加,K值降低,R15值增加,两组间比较,差异有高度统计学意义（P〈0.01）。ICG R15值、ICG K值与肝脏硬化程度间有显著的相关性,ICG R15值与肝脏硬化程度间呈显著正相关（r=0.792,P〈0.01）;ICG K值与肝硬化程度间呈显著负相关（r=-0.724,P〈0.01）。随着肝功能分级越严重,MELD评分越高,肝功能损害越严重。MELD评分与肝脏纤维化程度呈正相关,差异有高度统计学意义（r=0.842,P〈0.01）。结论慢性肝病患者的肝脏储备功能随患者肝纤维化程度的增加而降低,对肝纤维化程度的检测有助于衡量慢性肝病患者肝功能贮备的情况。

Correlation analysis of liver function reserve and hepatic fibrosis in patients with chronic liver disease

Objective To study the correlation of liver function reserve and hepatic fibrosis in patients with chronic liver disease,so as to provide the reference for clinic.Methods The clinical data of 48 cases of patients with chronic liver disease in our hospital was analyzed retrospectively.All the patients were divided into chronic hepatitis B group and hepatitis B liver cirrhosis of compensatory period group,with indole cyanogen green(ICG) discharge tested,liver stiffness detection,liver tissue fibrosis qualitative analyzed and model end-stage liver disease(MELD) score between the two groups.Results With the increase of the degree of liver cirrhosis,K value was lower,R15 value was increased,the difference was statistically significant between the two groups(P < 0.01).ICG R15 value,ICG K value and liver sclerosis had a significant correlation between the degree(r = 0.792,P < 0.01),between the ICG R15 value and liver sclerosis degree,there was a significant positive correlation between them(r = 0.724,P < 0.01);between the ICG K value and liver cirrhosis degree,there was a significant negative correlation between them.With the more serious liver function classification,the MELD score was higher,the liver function damage more serious.MELD score and liver fibrosis was positively related degree,the difference was statistically significant(r = 0.842,P < 0.01).Conclusion Liver function in patients with liver fibrosis with reserve reduce with the increase of the degree in patients with chronic liver disease,the degree of hepatic fibrosis patients with chronic liver disease detection can help to measure the liver function reserves.
