低分子肝素治疗妊娠晚期羊水过少的效果

目的观察低分子肝素治疗妊娠晚期羊水过少的效果和安全性。方法 109例病人随机分为治疗组56例,对照组53例。治疗组采用低分子肝素5 000 U/d皮下注射,对照组采用丹参400 mg/d静脉滴注,两组同时静脉补液1 000 mL/d,7 d为1疗程。1疗程后复查羊水指数,并比较两组剖宫产率、产后出血量和新生儿窒息率。结果两组治疗后羊水指数均有所增加,但治疗组的羊水指数较对照组明显升高（t=8.09,P〈0.05）;治疗组的剖宫产率较对照组明显降低（χ2=11.56,P〈0.05）;治疗组产后出血量较对照组明显减少（t=2.82,P〈0.05）;两组新生儿轻度和重度窒息率比较差异无显著性（P〉0.05）。结论低分子肝素治疗妊娠晚期羊水过少效果优于传统的治疗药物丹参,可以显著增加羊水指数,改善妊娠结局。

EFFECTS OF LOW MOLECULAR HEPARIN ON OLIGOHYDRAMNIOS IN LATE PREGNANCY

Objective To observe the efficacy and safety of low molecular heparin（LMH） on oligohydramnios in late pregnancy. Methods This study consisted of 109 patients who were assigned to receive hypodermic injection of 5 000 U/d of LMH（treatment group,56 patients） or intravenous Dan Sen 400 mg/d（control group,53 patients）;intravenous fluid infusion of 1 000 mL/d was undertaken in both groups,with seven days being one course.Amnionic fluid index（AFI） was detected,percentages of caesarean section,asphyxia neonatorum and postpartum blood loss were compared between the two groups. ResultsUpon completion of the therapy,AFI increased in both groups,but more in the treatment group（t=8.09,P0.05）,the percentage of caesarean section was lower and postpartum hemorrhage was less in the treatment group than that in the control（χ2=11.56,P0.05） and（t=2.82,P0.05）,respectively.There were no significant differences between the two groups in terms of mild or severe neonatal asphyxia（P0.05）. Conclusion Low molecular weight heparin is superior to traditional medicine,Dan Sen,for the therapy of oligohydramnios in late pregnancy,which can notably increase AFI and improve pregnancy outcome.
