微弹簧圈结合LEO支架栓塞颅内宽颈动脉瘤的效果

目的探讨微弹簧圈结合可伸缩自膨式（LEO）支架栓塞颅内宽颈动脉瘤的临床效果。方法蛛网膜下隙出血经DSA或CTA证实为颅内宽颈动脉瘤病人14例（7例后交通动脉瘤,5例椎动脉瘤,2例基底动脉瘤）,1例选择先释放支架,微导管穿过支架网眼填塞微弹簧圈;2例先填塞动脉瘤,栓塞完全后再在瘤口释放LEO支架;11例选择先半释放LEO支架后填塞动脉瘤,待栓塞完全后,再完全释放支架。结果 13例宽颈动脉瘤均应用Remod微弹簧圈结合LEO支架完全栓塞,1例部分栓塞。结论微弹簧圈结合LEO支架治疗宽颈动脉瘤具有易操作、输送方便、支架进出外微导管顺利的特点,效果好。

THE EFFICACY OF MICRO-SPRING COIL CONBINED WITH LEO SELF-EXPANDING STENT FOR THE TREATMENT OF WIDE-NECK INTRACRANIAL ANEURYSMS

Objective To evaluate the clinical effectiveness of retractable self-expandable stent（LEO） with detachable coils to embolize wide-neck intracranial aneurysms（IA）.Methods Fourteen patients with wide-neck IA were confirmed by DSA or CTA,of whom,one was first released LEO stent,followed by obturating the micro-spring coil;two were given obturation first,upon completion of the procedure,LEO stent was released;11 were treated by staged release of LEO stent first,followed by obturation of the IA,the stent was then completely released.Results Of the 14 aneurysms,13 were fully embolized by using combination stent-coil technique,one was partly embolized.Conclusion The combination of micro-spring coil with LEO stent in the treatment of wide-neck aneurysms is of advantages of good outcome,easy to perform,convenient to carry,place and remove the stent.
