心理干预对64层螺旋CT冠状动脉成像病人的影响

目的探讨64层螺旋CT冠状动脉成像检查中心理干预重要性及获得最佳图像的影响因素。方法将182例行64层螺旋CT冠状动脉造影检查病人随机分为干预组（n=92）和对照组（n=90）。对照组检查前给予一般的指导和口服倍他乐克,由扫描技术员按CT机器扫描同步录音模式训练受检者呼吸和屏气;干预组检查前、中、后给予受检者系统心理护理干预,检查前训练呼吸（指导屏气训练）和口服倍他乐克。比较两组病人心理状况、检查过程心率波动〉15 min-1的发生率、检查成功率。结果干预组检查前心率、心理状态较对照组稳定;检查过程心率波动〉15 min-1的发生率较对照组低,检查成功率较对照组高,差异均有显著性（χ2=8.802、6.734,P〈0.05）。结论 64层螺旋CT冠状动脉检查中实施适当心理护理干预能有效减轻病人焦虑、恐惧心理,缩短检查时间,稳定心率,提高冠状动脉成像质量及检查成功率。

THE INFLUENCE OF MENTAL INTERVENTION ON PATIENTS UNDERGOING CORONARY ARTERY IMAGING BY 64-SLICE SPIRAL CT

Objective To explore the importance of mental intervention at 64-slice spiral CT coronary angiography and influencing factors of obtaining optimized pictures. Methods A total of 182 patients undergoing 64-slice spiral CT coronary angiography were randomly divided into intervention group（92 cases） and control group（90 cases）.Patients in the control group received general guidance and were given oral Betaloc,and trained by a technician to match their respiration to the test equipment;to those in the intervention group,systematic mental nursing was offered before,during and after the examination as well as instruction of respiration and oral Betaloc.The psychologic status,the incidence of fluctuation of heart rate15 min-1 and the achievement ratio of examination between the two groups were compared. Results Before examination,the heart rate and mental status of those in intervention group were more stable;during examination,a lower incidence of heart rate fluctuation15 min-1 and a higher achievement ratio than those in the control group（χ2=8.802,6.734;P0.05）. Conclusion A proper mental intervention at 64-slice spiral CT coronary angiography can effectively relieve patient＇s anxiety and fear,shorten the time of CT scanning and stabilize heart rate,raising the quality of the image and the success rate of the examination.
