多层螺旋CT在小肠疾病中的应用

目的：探讨多层螺旋CT（MSCT）对小肠病变的诊断价值。方法：对34例疑有小肠病变的患者行多层螺旋CT平扫及增强检查。检查前清洁肠道，肌注盐酸山莨菪碱10-20mag以减少肠蠕动，口服足量2．5％等渗甘露醇水对比剂充盈肠管后扫描，结合多平面容积重建（MPVR）和／或最大密度投影（MIP）后处理观察图像。结果：正常12例；病变22例，均经手术病理证实：小肠肿瘤12例，其中间质瘤4例（良性1例，恶性3例），淋巴瘤2例，腺癌3例，腺瘤2例和脂肪瘤1例；Crohn病3例；术后粘连性肠梗阻3例；肠系膜扭转后梗阻2例；肠系膜上动脉栓塞1例；增生性肠结核1例。结论：多层螺旋CT对小肠病变的定位、定性、肿瘤分期、肠系膜及其血管病变有较高临床价值，是诊断小肠疾病的重要方法。

Application of multi-slice spiral CT in small bowel lesion

Objective： To study the diagnostic value of the multi - slice CT （MSCT） for the small bowel lesion. Methods： 34 cases with suspected small bowel lesion were examinated MSCT and enhanced CT examination. Those patient were cleaned intestine, injected 10 - 20mg anisodamine in order to reduce peristalsis, and adequately drunk 2. 5% isotonic mannitol in order to fill intestine. All images were observed by combining with multiplanar volume reconstruction （MPVR） and/or maximum intensity projection （MIP） image post - treatment observation. Results： 12 cases didnt find small bowel hsion; 22 cases of lesions were confirmed by surgery and pathology in which 12 cases were small bowel tumors, 4 cases were stromal tumors, lease was benign and 3 cases were malignant, 2 cases were iymphoma, 3 cases were adenocareinoma, 2 cases were adenoma and 1 case was lipoma; 3 cases were crohn disease, 3 cases were adhesive intestinal obstruction after operation, 2 cases were obstruction of mesenteric torsion, 1 case was superior mesenteric artery embolus, 1 ease was hyperplasia intestinal tuberculosis. Conclusion： MSCT is the diagnostic methods of small bowel lesion because there has high clinical value in location, qualitation, tumor stage, mesenteric vascular disease.
