肝源性溃疡临床特征及治疗探讨

目的　探讨肝源性溃疡 (HU)临床特征并观察小剂量心得安对HU的临床疗效。方法　观察 96例 (其中HU64例 ,单纯性消化性溃疡 3 2例 )受试者发病年龄、溃疡部位、HP感染率、出血率及治疗后溃疡愈合率、腹痛平均消失时间。将 64例HU患者随机分为A组和B组各 3 2例 ,单纯性消化性溃疡 3 2例为C组 ,三组均服用雷尼替丁15 0mgBid(早晨及睡前服用 ) ,A组联用心得安 10mgTid ,B、C组联用维生素B6 10mgTid ,疗程均为 4周。结果　HU发病年龄较单纯性消化性溃疡晚 15年、溃疡部位以胃溃疡及复合溃疡居多、合并出血率高 ,心得安可明显提高HU溃疡愈合率 (P <0 .0 5 )、总有效率 (P <0 .0 1)及缩短腹痛平均消失时间 (P <0 .0 1) (与B组比较 )。结论　HU溃疡部位以胃溃疡及复合溃疡居多 ,而且容易并发出血。在制酸基础上联用小剂量心得安有助于溃疡愈合及缩短腹痛平均消失时间

Hepatogenic ulcer:Clinical characteristics and managements

Objective The clinical feature of hepatogenic ulcer was observed and its management with light does of propanolol tries.Methods 64 patients with hepatogenic ulcer were divided at random into group A(n=32) treated with propanolol 10mg Tid Ranitidine 0.15 Bid and group B(n=32) with Ranitidine 0.15 Bid,and meanwhile group C (n=32) of patients with peptic ulcer managed by Ranitidine 0.15 Bid was enrolled as another control.All 96 patients' age of occurring,clinical expression distribution of ulcer,rate of HP infection ulcer bleeding and ulcer healing was recorded.Results In general hepatogenic ulcer occurred at an age of 15 years older than the peptic,with a distribution solely within stomach and compound ulcer and a higher risk of bleeding.The use of propanolol produced a higher rate of ulcer healing (P<0.05),as well as higher rate of total effctive (P<0.01),and a lower mean time of alleviation on abdominal pain in group A (P<0.01) than those in group B.Conclusions Hepatogenic ulcer with a distribution solely within stomach and compound ulcer and a higher risk of bleeding,based on the treatment of acid inhibition in company with light dose of propanolol would be benefit to ulcer healing and the mean time of alleviation on abdominal pain would be lower.
