48例腹型过敏性紫癜临床特点及胃镜结果分析

目的:探讨儿童腹型过敏性紫癜的临床特点及胃镜表现,为早期诊断提供依据。方法:回顾性分析48例腹型过敏性紫癜患儿的胃肠表现及胃镜资料。结果:48例患儿均有腹痛,腹痛部位不固定,症状严重而腹部体征轻微,14例伴有便血。胃镜检查见胃肠黏膜不同程度的充血、水肿、糜烂、点片状出血、多发浅溃疡,越接近十二指肠降部黏膜改变越重,食管黏膜正常,未见胃溃疡。解痉剂和抑酸治疗无效,糖皮质激素治疗有效。结论:腹型过敏性紫癜的胃镜下粘膜改变具有一定特征性,早期胃镜检查可有助于腹型过敏性紫癜的确诊,及时合理治疗。

Analying the clinical characteristics and the results from endoscopy of 48 cases HSP

Objective:To investigate the children Henoch-Schonlein Purpura(HSP) clinical characteristics and endoscopy results so as to provide a guide for early diagnosis.Methods:By retrospectively analyzing the gastrointestinal symptoms and endoscopy results of 48 cases.Results:48 cases all have the symptom of bellyache,while the ache spot is not fixed.The symptom is serious,but there is a slight abdomen physical sign.Among them,14 cases have blood in stool.Besides,through endoscopy,the mucous membrane shows various degrees of hyperaemia,edema,gedaucjed,dot-or plate-shaped blood,frequent-bout superficial ulcer;and the mucosal lesions become more serious as endoscopy is more closer to the descending duodenurm.The oesophageal mucous is normal and there is no case with Gastric Ulcer.In addition,antistamodic and acid-suppressing agent is of no use in these cases,but hormonotherapy is useful.Conclusion:The mucous membrane lesion of HSP has its characteristic.Early endososcopic observation can help to diagnose HSP early so that the effective treatment can be provided in time.
