26例癌性结肠梗阻的临床分析

目的　探讨结肠癌性肠梗阻的诊断及治疗。方法　对结肠癌性肠梗阻的诊断、手术治疗的术前准备、手术时机、手术方式选择进行了分析。结果　本组术前能确诊的结肠癌 1 1例 ,以“腹痛待查”诊断者 1 5例。 1 2例右半结肠癌行一期右半结肠切除 ,1 4例左半结肠癌 ,也行左半结肠切除 ,其中 1 0例一期吻合 ,3例横结肠造瘘 ,1例Hartmann手术 ,2例死亡。结论　结肠癌性肿块并肠梗阻诊断一经确诊 ,应争取一期切除吻合。术中结肠减压及灌洗是左半结肠癌安全进行结肠一期切除吻合的重要措施

Clinical Analysis on Carcinomatous Obstruction of Colon

Objective To probe into the diagnosis and thetreatment of carcinomatous obstruction of colon. Methods The diagnosis of carcinomatous obstruction of colon, the preoperative preparation, the opportunity of operation and the choice of the operative way were analyzed. Results Therre were 11confirmed diagnosis cases with carcinoma of colon and 15 cases to be investigated because of abdominagia; The twelve cases with right half carcinoma of colon were operated with the removal of primary right half colon, and the fourteen cases with left half carcinoma of colon were operated with the removal of left half colon. Among them therewere 10 cases of first anastomosis, 3 cases of transverse colostomy, one case of Hartmann operation and 2 cases of death. Conclusion Once the carcinomatous obstruction of colon is confirmedly diagnosed, the patient should be operated withthe primary removal anastomosis The decompression of colon and lavation are important safe measueres for the removal of colon in the left half carcinoma of colon.
