髋关节脱位伴髋臼骨折的手术治疗

目的探讨髋关节脱位伴髋臼骨折的手术治疗方法。方法自2006年1月-2011年4月收治髋关节脱位伴髋臼骨折33例,其中资料齐全28例,手术治疗26例,男19例,女9例,年龄16-65岁,平均年龄35.5岁,右髋12例左髋16例。以改良的Merled’Aubigne-Poxtel评分系统评价。结果术后平均随访24月﹙11-33月﹚,安照改良的Merled’Aubigne-Poxtel评分系统评价26例手术患者中优6例,良10例,可4例,差6例,优良率61.5%。髋关节脱位在24小时内复位的共15例,优良率为79.7%﹙12/15﹚,超过24h复位的,优良率为38.4%﹙5/13﹚。结论髋关节脱位伴髋臼骨折治疗要求及时纠正脱位、尽早手术重建骨折的髋臼,恢复髋关节的稳定性,早期不负重功能锻炼,减少并发症的发生。

Surgical treatment of disloction of the hip with acetabular fracture

Objective To explore the method for the surgical treatment of disloction of the hip with acetabular fracture.Methods 33 cases of disloction of the hip with acetabular fractures were treated from Jan 2006 to April 2011.,among which 28 patients were followed up,and from which 26 cases﹙19 male and 9 female﹚were treated with open surgery,The average age 35.5 years﹙16-65﹚.The 26 disloction of the hip with acetabular fracture involved 12 on right side,16 on left side.All the 26 cases were evaluated with Revised Merled Aubigne-Poxtel grading system.Results The average follow-up time wes 24 months ﹙from the range of 11 months to 33 months﹚.According to Revised Merled’AubignePoxtel grading system,In 26 patients,the outcome was followed： excellence were 6 cases,good 10 cases,common 4 cases,bad 6 cases;the good adds to excellent rate was61.5%.15 cases of disloction of the hip were reduction within 24h,the good and to excellent rate was79.7%﹙12/15﹚,and the others reduction exceed 24h,the good and to excellent rate was 38.4%﹙5/13﹚.Conclusion The treatment of disloction of the hip with acetabular fracture is that the dislocated hip should be reduced in time and acetabular fracture be reconstructed as early as to resume the stability of the hip and early functional exercise without weight bearing to reduce complications.
