腹腔镜、结肠镜双镜联合在结直肠手术中应用13例报告

目的探讨纤维结肠镜和腹腔镜双镜联合在结直肠疾病手术中的应用意义。方法回顾对13例腹腔镜定位困难的结肠病变,用纤维结肠镜定位、标记,经腹腔镜完成手术操作。结果13例均顺利完成定位和手术。肠段切除平均用时130(80~190)min,结肠癌根治术平均用时170(140~230)min,术中平均出血量50(20~100)ml,肠道运动恢复时间44(18~52)h,术后平均住院6(4~10)d。结论纤维结肠镜和腹腔镜双镜联合术中定位准确、创伤小,可提高腹腔镜手术成功率,值得推广。

Clinical Application of Combined Treatment with Laparoscopy and Colonoscopy to 13 Cases of Colonic Diseases

Objective To explore the clinical application of combined treatment with laparoscopy and endoscopy to colonic diseases.Methods Thirteen patients with colonic diseases who couldn't be oriented with laparoscope were oriented with endoscopy and operated under laparoscopy.Results Their diseases were successfully oriented and operated. The average operative time for laparocolectomy and radical colonic excision was 130(80-190) minutes and 170(140-230) minutes,respectively.The average blood loss was 50(20-100) ml.The average time for gastrointestinal function recovery was 44(18-52) hours.The average hospitalization day was 6(4-10) days.Conclusion It has a precise orientation and slight injuries that the combined treatment of laparoscopy and colonoscopy is applied to the colonic diseases when they can not be oriented precisely in laparoscopy.It is an effective and feasible way to colon operation.
