The clinical characteristics of 35 cases of brucellosis

Objective To summarize the clinical characteristics of 35 patients with brucellosis for improving early diagnosis and treatment of brucellosis.Methods The clinical characteristics of 35 cases of brucellosis diagnosed in the Second Affiliated Hospital of Nanchang University from January 2013 to October 2019 were analyzed retrospectively.Results The included patients were mainly farmers(62.9%,22/35)and middle-aged(41-65 years old,57.1%).Most(80.0%)of the patients had a clear history of contact with cattle and sheep.The main clinical manifestations were fever(97.1%),fatigue(85.7%),muscle and joint pain(68.6%),chills(48.6%),hyperhidrosis(40.0%)and low back pain(34.3%).White blood cell(WBC)count was normal in 29 cases(82.9%).Eosinophil percentage decreased significantly in 26 cases(74.3%)and hemoglobin decreased in 23 cases(65.7%).Liver function was abnormal in 28 cases(80.0%).C-reactive protein increased in 33 cases(94.3%).Erythrocyte sedimentation rate increased in 23 cases(85.2%).Procalcitonin increased in 2 cases(6.5%).Conclusions The clinical symptoms of 35 cases of brucellosis are long-term fever,fatigue,and muscle&joint pain.Laboratory tests include normal WBC and procalcitonin levels,significantly decreased eosinophil percentage,decreased hemoglobin,increased C-reactive protein and erythrocyte sedimentation rate,and high incidence of liver impairment.Doxycycline-based combination therapy is appropriate for good treatment outcome.
