新型C1侧块-C2椎弓根螺钉置钉导向器的临床应用

摘    要:目的 评价新型C1侧块-C2椎弓根螺钉置钉导向器辅助置钉的有效性和安全性.方法 2017年7月—2019年9月,64例患者在海军军医大学长征医院接受C1,2重建.其中16例由经验较丰富医师在导向器辅助下完成置钉(A组);16例由经验较丰富医师徒手置钉(B组);16例由经验较少医师在导向器辅助下完成置钉(C组);16例由经验较少医师徒手置钉(D组).记录并比较各组置钉时间和螺钉等级.结果 A组置钉准确率高于B组,差异有统计学意义(P<0.05);2组置钉时间差异无统计学意义(P>0.05).C组置钉时间低于D组,准确率高于D组,差异均有统计学意义(P<0.05).经验较少医师采用导向器辅助置钉的学习曲线趋于稳定的手术次数分别为12次(C1)和13次(C2),经验较丰富医师分别为12次(C1)和10次(C2).结论 新型C1侧块-C2椎弓根螺钉置钉导向器辅助置钉安全有效,适用于野战和急救等医疗条件有限的极端情况.

Abstract:Objective To evaluate the efficacy and safety of novel C1 lateral mass-C2 pedicle screw insertion navigation system. Methods From July 2017 to September 2019, 64 patients underwent C1,2 reconstruction in Changzheng Hospital of Navy Medical University. Among them, 16 patients were completed screw placement guided by navigation system by experienced doctors(group A);16 received screw placement by free hand by experienced doctors(group B);16 did screw placement guided by navigation system by less-experienced doctors(group C);16 did screw placement by free hand by less-experienced doctors(group D). The screw placement time and screw grade of each group were recorded and compared. Results The accuracy rate of screw placement of group A was higher than that of group B, and the difference was statistically significant(P < 0.05);and there was no significant difference in time of screw placement between the 2 groups(P > 0.05). The time of screw placement in group C was lower than that in group D, and the accuracy in group C was higher than that in group D, and the differences were statistically significant(P < 0.05). Learning curves showed that the execution time reached a steady state after 12(C1) and 13(C2) times for the less-experienced doctors, and 12(C1) and 10(C2) times for the experienced doctors. Conclusion The novel C1 lateral mass-C2 pedicle screw insertion navigation system is safe and effective, and is suitable for extreme cases with limited medical conditions such as field operations and emergency treatment.
